AI medical record review and medical chronologies for claims teams and law firms

Turn thousands of pages of medical records into a dated, cited chronology in a fraction of the reading time. Every entry links to its source page, gaps and red flags are called out, and your people review it before anyone relies on it.

  • Every entry linked to its source page
  • Built by engineers who have done this in production
  • BAAs signed; processing in a secure cloud

Thousands of pages. One deadline.

Medical records arrive as stacks of scans, faxes and duplicates, in no particular order. Someone has to read every page before a reserve is set, a claim is decided or a demand goes out.

  • Nurses, adjusters and paralegals spend hours reading instead of deciding
  • Duplicates, out-of-order pages and handwriting slow every file down
  • A missed gap in treatment or prior condition changes the outcome of a case
  • Backlogs grow every time record volume spikes
  • Summaries vary from one reviewer to the next

Built for both sides of a medical claim

The same records, read the same careful way, for the people who handle and argue claims.

  • For claims teams

Adjusters, nurse case managers, TPAs and carriers

Know what happened, when, and what it cost, before you set reserves or decide a claim.

    • Faster file review for workers' compensation and liability claims
    • Preparation for independent medical exams and utilization review
    • Treatment gaps, prior conditions and inconsistent histories flagged
    • Medical bills summarized alongside the treatment timeline
    Talk about your claims volume
  • For law firms

Personal injury and workers' compensation attorneys

Evaluate cases faster and build demands and depositions on a timeline you can cite page by page.

    • Case evaluation before you take a matter on
    • Medical chronologies for demand letters and settlement packages
    • Deposition and expert preparation with every fact cited
    • Treatment gaps and pre-existing conditions spotted before opposing counsel does
    Talk about your caseload

What you get for every case file

Delivered in the formats your team already works with: Word, Excel or a bookmarked PDF.

  • A dated medical chronology

Every encounter in order: date, provider, facility, complaint, diagnosis and treatment.

  • A citation for every entry

Each line links to the exact source page, so anyone can check it in one click.

  • Gaps and red flags

Treatment gaps, prior conditions, inconsistent histories and missing records, called out for review.

  • A case summary

A plain-language overview of the injury, treatment and current status.

  • A medical bills summary

Charges by provider and date, alongside the treatment they relate to.

  • Organized, de-duplicated records

Duplicates removed and pages sorted and bookmarked by date and provider.

From a stack of records to a cited chronology

Six steps, with a person reviewing before anything is relied on.

  1. Receive the records

Scans, faxes, PDFs and exports, uploaded securely or pulled from your claims or case system.

  2. Read every page

OCR turns each page into text; page images are kept so tables and layout are not lost.

  3. Sort and de-duplicate

Pages are classified by type, grouped by provider and date, and duplicates removed.

  4. Extract the encounters

Dates, providers, diagnoses, procedures, medications and charges, each tied to its source page.

  5. Flag what matters

Gaps in treatment, prior conditions and inconsistencies are flagged; uncertain values go to review.

  6. Review and deliver

Your reviewer, or ours, checks the draft and the chronology is delivered in your formats.

AI does the reading. Your people make the calls.

We don't sell AI that decides claims or cases. It removes the hours of reading, so experts spend their time on judgment.

The AI does the reading

  • Reads and sorts every page, however long the file
  • Extracts each encounter with its source page
  • Flags what a reviewer should look at first

Your people make the calls

  • Nurses and adjusters judge treatment and causation
  • Attorneys decide case strategy
  • Nothing is relied on until a person has reviewed it

Health information handled with care

  • A BAA signed where protected health information is involved
  • Processing in your cloud environment or a secure, access-controlled environment of ours
  • Pilots run on de-identified case files
  • No health information accepted through our website
  • A full audit trail of every file, page and change
  • Personal data masking where records are shared beyond the case team

Built by engineers who've done this in production

  • Team experience

Our AI engineers have built systems that turn thousands of pages of patient records into a medical chronology, with each entry traced to its source.

  • Integriti result

Our document AI runs in production inside a claims platform, linking every extracted value to its source page and sending uncertain values to a person.

Two ways to work with us

  • Built into your systems

We build the pipeline into your claims platform or case management system, in your cloud.

Best forCarriers, TPAs and larger firms with steady volume

  • Run for you as a service

Send case files securely and receive reviewed chronologies back, with no system to install.

AI medical record review FAQ

Is this a nurse review service?

No. We build the AI that reads the records and drafts the chronology, with every entry linked to its source page. Your nurses, adjusters or paralegals review it and make the calls. That's why it scales across thousands of pages without adding headcount.

How accurate is it?

It depends on your records: scan quality, handwriting and how files are organized. We measure accuracy on a de-identified sample of your case files in a pilot, so you see the results before committing. The pilot has a fixed scope and fixed fee, credited toward the full build.

Which records can it read?

Typed and scanned medical records, including clinic and hospital notes, imaging and lab reports, prescriptions and medical bills. Handwriting and poor scans are flagged for a person to check instead of guessed.

How do you handle patient health information?

Processing runs in your cloud environment or ours under a BAA, with access controls and a full audit trail. We never accept health information through our website, and pilots use de-identified files.

Will it replace our medical reviewers?

No. It removes the hours of reading and sorting so your reviewers spend their time on judgment: causation, treatment questions and case strategy.

Can it fit into our case or claims system?

Yes. We can build it into your claims platform or case management system, or deliver chronologies as a service.

Send us a de-identified case file

Share one representative file and we'll show you the chronology our AI produces, with every entry cited. Or start with a 30-minute call with our CEO or CTO.

  • A cited chronology from one de-identified file
  • The gaps and flags it finds
  • What it would take to run on your volume

Never send files containing patient information through this form

After a short call, we'll send you a secure upload link.
